Special Request for Admittance
This form is used to appeal an application denial for admission due to a suspension at another college/university.

Medical Assistant
The Fall 2023 Medical Assistant application will open January 9, 2023 and close on May 31, 2023 at 11:59pm. Nursing Program

OPCA Clinical Applications
The Fall 2023 application will open on January 03, 2023 and will close on May 01, 2023 at 11:59pm. Go to the OPCA website for application instructions and the online application. Paramedic Fire Science
The Fall 2023 Paramedic Fire Science application is open until the start of Fall semester. Once filled, qualified applicants will be added to the waitlist for Fall semester.

Radiologic Technology

Course Evaluation Appeal
This form is used by students who wish to appeal transfer credit evaluation decisions. Refer to ‘Your Rights as a Transfer Student’ for more information.

Credit by Exam (Credit for Prior Learning)
This form is used by students who wish to earn credit via course-specific examinations. Students must consult with theCredit for Prior Learningspecialist to review qualifications.

Getting Started (with Credit for Prior Learning)
This form is used by students who want to explore their many options for earning credit from learning accomplished through life and work experiences.

 

Authorization for the Release of Student Information
This form is used by students who wish to authorize Century College to release and/or discuss information related to their financial and academic record to a person of their choosing.

Change of Income
Use this form when your 2022 income is substantially less than your report 2020 income.

Childcare Grant
This form is used by students who wish to apply for the childcare grant program.

Consortium Agreement
This form is used by Century students to take a course at another Minnesota State institution and use their Century College financial aid to help offset the educational expenses.

Estimated Refund Calculation Worksheet
This worksheet is used by students who want to estimate how much money they may receive via financial aid and/or third party funding.

FAFSA Signature
This form allows you to sign your FAFSA using a paper signature form.

FAFSA Signature with Step 4-Parent Information
This form allows for the collection of parent FAFSA information and parent signature when a FAFSA was submitted in error as an independent student.

Student Document Submitter
Securely upload your Financial Aid documents to the Financial Aid Office by using the Secure Uploader. Some documents include Financial Aid Letters, Tax Forms, Child Care Grant Applications, Maximum Timeframe Appeals, etc. Submitted documents can take up to four weeks for processing. Any updates or requests for more information will be sent to your Century College email.

Student Loan Change Form
This form can be used to reduce or cancel any loan requests which have been made for loans that have not yet been disbursed.

Authorization for the Release of Student Information

Change of Income

Course Audit

Duplicate Diploma Request
This form is only for students that need a replacement degree document. The diploma order takes approximately 4 to 6 weeks. A transcript is easily accessible and is the most official record of your graduation.

Exceed Maximum 19 Credit Limit Appeal
This form is used by students who wish to take more than 19 credits in a single semester.

High School to Undergraduate Student
This form is for students who have previously taken courses at Century College through PSEO, Supplemental Enrollment, Century in the High School, Early/Middle College and High School Student in Transitions programs and who are graduating from high school and now intend to continue as an undergraduate student at Century College.

Immunization
This form is used by students who need to provide proof of immunization. Minnesota law requires that students born after 1956 must provide proof that they have been vaccinated against certain diseases. NOTE: To ensure the health and safety of our campus community, Century College is following guidance from the Center for Disease Control (CDC), the Minnesota Department of Health (MDH), and the Minnesota State System Office. 

Minnesota Financial Aid Eligibility and/or Residency
This form is used to be considered for resident tuition rates.

Preferred Name Request
Complete this form if you would like Century College to use a preferred name for you in your campus email address, system directories, class rosters, and other Century College records where possible.

PSEO Authorization for the Release of Student Information
This form is used by PSEO students who wish to authorize Century College to release and/or discuss information related to their financial and academic record to a parent/guardian.

Repeat Course
This form is used by students who wish to repeat a course.

Restricted Course Waiver
This form is used to waive a course restriction, based on completion of similar credits taken elsewhere.

Senior Registration/Course Audit
This form is used by senior citizens (62 years or older) who wish to register or audit a course.

StarID Reset

Student Change of Information (change/add major or programs)
This form is used for students to change information such as major, program, name, and/or social security number.

Transcript Request
This form is used to request an official transcript. Students can also print unofficial transcripts through eServices.

Visiting Student Pre-requisite Verification
This form is used to verify that visiting students have met course pre-requisite(s).

Grade Appeal Form
Students may appeal a final course grade according to the Grade Appeal Policy by following the steps of the Grade Appeal Procedure. Complete the Grade Appeal Form and send it to the appropriate dean to initiate the appeal.
